What to have ready
Having these ready makes your enrollment call faster. If we ask for documents, send copies, never originals.
- Social Security numbers for everyone applying. Lawfully present immigrants who need coverage can use their immigration document numbers instead.
- Income for everyone in your tax household, such as pay stubs or W-2 forms, and your best estimate of your 2027 household income.
- Your current health insurance, if you have any: the company and plan.
- Job-based coverage offered to anyone in your household, even if you don’t take it.
- Proof of identity, date of birth and citizenship or immigration status if asked, such as a U.S. passport, birth certificate, green card or employment authorization card.
